I am writing to follow up on previous communications relative to the closure of my practice, Renaissance Primary Care and Wellness Group LLC, and to provide information regarding how you can follow up with me if you require copies of your medical records. I notified all of my patients on January 2, 2011 via an email and per a letter sent via US mail. Unfortunately, it has come to my attention, several patients were not aware of the practice closure.
I want to thank all of you for allowing me to be the caretaker of your health. If any of you are in need of copies of your medical records, please send a written request to Renaissance Primary Care & Wellness Group, LLC, PO Box 363, Savage MD 20763 with a $20 check made out to Renaissance Primary Care & Wellness Group, LLC, to cover administrative and handling costs. Please indicate in the written request whether you would like to have the records provided on CD or via paper copy. The default method of providing the records will be CD as it is more efficient, and allows your new provider to download the records quicker in a more environmentally friendly manner. I will do my best to respond to your request within 15 business days. Please keep in mind that I do not have the benefit of an office infrastructure to support these requests and therefore I am providing the records with my home resources. If you have requested records in the past by phone or fax, please keep in mind that the office has been closed since Jan 31, 2011 there is no fax number or phone and any and all requests received have been processed. If you are emailing, faxing, or trying to call in your requests, your records requests will not be received or honored. Medical records retrieval from Renaissance Primary Care is an official process as noted above and is separate from establishing care with me at Johns Hopkins Community Physicians. Your patience is appreciated in this process.
Please be assured that for those of you who do not need your records, you records are being housed on a secure sever with the electronic medical records vendor. This will remain so for the next 5 years until Jan 27, 2016, when they would be destroyed in compliance with HIPAA. Previous lab results and imaging studies can all be obtained by your new provider from the lab and/or radiologist where your studies were done if you do not wish to purchase your medical records.

As of Dec 2nd I will be in a permanent position with Johns Hopkins Community Physicians, Fulton Location. The office opened in May 2013.
